Transitional Care Management

Transitional Care Management (TCM) consist of services directed to patients after hospitalization and focus on addressing the hand-off period between inpatient and community setting.

In this program, the practitioner accepts care of the beneficiary post-discharge from the facility, settling without a gap.

This means that this service should be provided during the following 30 days after the patient is discharged from the hospital.
TCM can be provided in face-to-face visits or non-face-to-face services.

This type of care also can open the door to providing other services, such as CCM, and is complementary to it for complex patients.
