Remote Patient Monitoring

Remote Patient Monitoring (RPM) Refers to the collection and analysis of patient physiologic data to identify real-time health status changes and modify plan treatments related to chronic or acute conditions.

Some parameters measure our weight, blood pressure, and blood glucose. This information has to be collected automatically, not self-reported by the patient.

Medicare has been incentivizing this program since 2020 since it allows closer monitoring of vulnerable patients, and there is a good reason behind that; RPM has been shown to reduce readmission, shorten hospital stays and improve quality of life at a lower cost.

RPM allows faster interventions, promotes self-management by the patient, and promotes more efficient coordination of care.

Most of the activities related to RPM can be furnished by clinical staff under general supervision, allowing you to take care of your patients with the most reliable information.
